How do you all keep track of what items are good for what champs?

I recommend that you use OPGG or any similar website for that. They usually show the most commonly used builds for specific champions within that patch. If you're not convinced yet, there's also Probuilds, which shows you what professional players are doing.

There are companion apps like Blitz, Mobalytics, and Facecheck that give item recommendations.
